Application Documents
Documents to be completed by all operations
- Application Form
- Certification Agreement (to be signed and witnessed and returned with the application forms)
Documents to be completed for the Producer Module.
- Producer Management Plan
- Statutory declaration Producer Form
- Non-Certified Planting Material Declaration
- Transport Declaration
Documents to be completed for the Processor Module.
To apply for certification:
- Please complete the Certification Application Forms as appropriate and return them to our office along with the sub-license agreement from Certified Sustainable. Note: you must fill out every section of each form or write N/A (where applicable), we cannot accept documentation that is left blank.
- Once these documents have been received by the office, our Accounts Department will issue an invoice that must be paid prior to the progression of your application.
Please note that the Statutory Declaration requires initials and dating at the bottom of each page, plus full signatures of an eligible witness and yourself. Please ensure that you sign and have witnessed the appropriate documents were indicated.

Please email info@aco.net.au for any new Product, Ingredient, Input, Planting Material Use and Label Requests. Please ensure the email subject line contains “Certified Sustainable” and the type of request.
